Program for data to update on another worksheet in same workbook

I am trying to program this Direct Dispute tab.When send it selected. The form will clear all data fields and write the information to the Roll up Tab.

Each tine Send it selected it will populate the Roll up tab with a new record.

remove L4:M6 so that it should look like this:-

Your formula will thus be left intact.
