Hello everybody this is my second attempt in building a user form in excel and I want the form to pop up automatically when excel is open and macros is enable which I have done before and I used the code below but for some reasons its not working on this user form ?? Any help please

I tried that code as well but nothing working ??
Did you put the code inside WORKBOOK VBA Page?
It should run the macro everytime workbook opens, and the macro is MyFormName.show
If doesn't work, could you please post the workbook here or a sample?
I tried both codes but still not working here is the workbook.. again I am new at this its my second time Patient Wellness Form.xlsm
