    User form to create PDF document

    Hi, i was wondering if it is possible to have a user form that can create a barcode (Code 39 / Code 128) on a pre made PDF document that i can email to customer for returns purposes. Attached is an example of how i would like the PDF document to look like when the "RMA" number is entered into a text box in a user form.
