Copy certain cells into list on different sheet if certain condition is met

I have a workbook in which the first sheet contains a list of names, and enrollment statuses. I want to be able to have columns C through F (which contain first name, last night, DOB, SSN) copy to sheet 2 in a list only when the enrollment status (indicated in column A) is either "Enrolled- Not Primary Care" or "Enrolled- Primary Care".

Is there a way to do this?
