Import Data from Another Workbook in Same Folder with Multiple Lookup Criteria

I am trying to import data from a separate (database) workbook that is located in the same folder as the working (user) workbook without having the user having to open the database. The user workbook has two criteria that I would like to use in grabbing data from the database workbook. The first criteria is the worksheet name in the database workbook. The second criteria is the set of data desired for import. I have attached an example of both the workbooks. Let me know if additional info is needed. Thanks in advance for the help.

Looks good LeoTaxi! What if I had an extra column at the end in the database but didn't want to include it in the import?
