Customized from Template

Hi,

I currently have 2 files that I need to use to create 100 customized survey forms:

(a) a list of 100 names; and,
(b) an excel template to be personalized with the names listed in (a)

Can someone point me to some VBA code that I can use to create 100 customized macro-enabled files from the 2 files above?

Note: It is important that the 100 customized files contain the same userforms and vba modules as the original template "(b)". My prior attempts using some other vba code has only made copies of the worksheet from the template, but the copies didn't contain the "behind the scenes" userforms or vba modules, and so they don't function as intended.
