VBA Code for Index/Match/Match to input into excel based on 2 criteria inside form

hiya all
this is my first post so bear with me

I have a worksheet that has in column A a list of names, column B their respective payroll numbers, column C their crew and Column D their dept
the rows consist of dept types with their own functions

ive created a form which when selecting a name, pulls through their details from columns B,C and D
with this form I would like to input a date and function and the form to input that date based on the criteria of function and payroll number
(please see attached)

im fairly new to VBA so any help would be appreciated!

You don't really give enough information - like sheets that you are using, etc. But, you can write out values to a specific sheet using code like this:
