Checkbox labels using cell values and formats

I have a checkbox where I'd like the label to change depending on it's status. When on, it should have a static value, when off it should change to a combination of text provided and formatted values from another worksheet. I can get the values to appear, but I cannot determine how to set the number formatting.

When the checkbox is un-selected, the result should be: "Using Actual Values 85% / 15%" The 85% and 15% come from Sheet2. When selected it should read "Using default Values 75% / 25%"
