    Extract data from options selected on form controls

    Hi

    I require some help with extracting the date selected from a radio button. I am not so familiar with form controls. On sheet1 tab, I have 2 selectable catergories, Spares and Emergency. An option of yes/No needs to be selected. When an input of the location number is keyed in cell B3, the date should move over to column H and I in the report tab. If yes was selected a tick to be shown and if no was selected a cross to be shown. I have attached the report for your kind assistance.
