Auto Fill Information

1. Is it possible to have a series of questions pop up when opening a workbook and have that information be filled in somewhere in the sheet?
2. Is there a way to make it impossible to bypass the questions without a password, or until the questions are answered?
3. Can it be setup to only ask the questions the first time the sheet is opened? (Maybe it only asks the questions if a certain cell is empty.)

I don't have a completed workbook yet. I would like the answers to show up on the second sheet
