Change the shape of option boxes to check box but only one one check (option) is allowed

Hello,
Is that possible to take option boxes but turn that into square checked boxes shape (with caption).
The option checked box is only allow one check. I need five option checked boxes.

Hi Rocky,

You are trying to change a standard that most people know. Radio buttons (also called option boxes) are round and only one can be selected in a group. Check boxes allow multiple selections. Read:

If this is at all possible, you would need to write code behind your Check Boxes to uncheck all the other check boxes if any one is checked.
I think you should follow the standard and not try to change it.
