Need help in creating a Check Sheet which use Data Entry

Hi guys.

I need help in creating a check sheet for my work. This check sheet use the format of Data Entry, we key-in the data and the save it on a different worksheet. Periodically, I want to export the saved data as a pdf file.

Please refer to my excel file for better understanding.
I have no knowledge on creating such program so I request the experts in here to show me the rope.
