    Fuzzy Lookup sequential order

I am attempting to use fuzzy lookup to compare find actual duplicates (same name, same person) rather than just duplicates that have the same name. Is it possible to have fuzzy lookup provide a similarity score in sequential order (from top to bottom) for a table? I want to receive a similarity score comparing two tables in the same row.

It would help if you upload sample raw tables and small sample of manually created expected output.

Have a read of link below, it may give you idea of how to use multiple columns for fuzzy lookup.
