transpose rows of data to text blocks

Good Evening,

I have an issue I have been struggling with. I need to transpose rows of data into rows, in a block format, with the header preceding every row. I guess this would be like taking addresses and phone numbers in a table and putting them into a label format, like Mail Merge in Word. However, it needs to be in Excel. Also not all tables have the same number of columns. This is what I am trying to get to. It gets tedious to do it manually when there are 5 or 6 columns and 40 to 50 rows of data. Your preview is not really clear. You can use something like this as a basis for transposing.
