Data Entry Form VBA Code

Hi, i'm new to vba and just watching youtube videos for tutorials. I was able to follow some but my save function is not working and the information is not being saved on the "Data" sheet. i copied the formula as shown in the tutorial but no luck. any one can help? here's the vba code i have.
