Create record program

Can someone help me to create record program?

Step:
1. In excel attachment, when I enter the all the required details Sheet 1(2) and click "Done" button, it will show the results on Sheet 1
2. After that, the programme will automatically clear all the details that I fill in before and just left the results.
3. So I can continue for new records.

Notes: Do not make the adjustment on layout that already fixed

Right Click On Your Sheet Name At the Bottom Of Excel and Select View Code
Paste this code in the module that opens and close it.

Thanks for your help. But I still face some problem on the latest file:
1. Only the date and amount had reflect in the results page which in Sheet 1. But the customer name, notes 1 and notes 2 didn't exists.
2. Old results shouldn't be clear if there are new result available. New result will shown below the old results.
3. In Sheet 1(2), all the information that are fill in don't required to clear all once click "Done"
