Macro to Insert 4 Columns from Column E and fill repeated values in inserted columns

Dear Friends,

I need to Insert FOUR new columns from Column E and paste values (like Previous column Header + Check1; Previous column Header + Check2, .....) in the inserted columns.

That is:

1) From Column E to H (no. of columns vary), there are column Headers (in row 3).
2) Insert 4 new columns "in-between those columns" and further replace with values (with "Concatenating Previous Column Header + Check1; Previous Column Header + Check2....) that are repeatedly to be pasted).

I have attached an Excel sample file for better understanding.

Please help me.
