    Exporting rows onto a form

    I have 300+ forms I fill out each month.
    The form contains some contact information at the top, followed by 10 yes/no questions, and ends with a comment section.

    It takes forever to individually copy and paste info into each one, and then save them.

    I found a VBA that will take each line of info and save it into an individual file, but I'm looking for it to export the information onto this pre-made form AND then save it.

    Does anyone have any ideas? Or an help in any way? I am a complete novice when it comes to macro/vba writing, but that's why we have forums like this! :)

    I attempted to attach the VBA code in this same window, but I was encountering an error.

    I assume you also have a file with the source data? I can't imagine that your file is exactly the same as the sample file available for download on the page you referenced. Also, I'd name your option buttons in a more intuitive way in the form, respect_yes and respect_no for example.
    Also, would the code be contained in the Form file? or the one with the source data (more logical).

    I attached a source file to the original post. From that original source file, I'd like each row to be transposed onto a form, and saved as an individual file.

    I'm aware some tweaks may need to be made to the actual form layout to make it work (The yes/no functions for example can be drastically simplified. The 10 questions each need a yes/no response. all 10=yes=pass, anything less=fail).

    Code would probably need to be in the source data, as the form would not ACTUALLY exist until you hit "run". And then the form would be created as a result of the vba.

    My apologies if my explanations are lacking, I'm trying very hard to put my thoughts into words that make sense!
