    I need a code to import information into a word on a form.

    I need help to take information from a excel sheet of specific columns to import to a word document.
    I have included a picture of both and files for testing. I need info form Column A and M to populate in Box 1 of the form. I can have anywhere from 20-50 a day. Then I need the word doc to be saved as separate file with persons name so I can email them the form.
