Only one check mark allows in a group of form control check box

Hello,
I need help on form control check box.

I want only one check mark allowed in a group of check boxes. When a check box is checked, then all other check boxes in that group will be unchecked.
There are three groups, each group works independently.

I want to match the style of the square check boxes from an existing form but using Excel.
I try to eliminate human error to check more than one check box.

You're basically flying in the face of Windows design principles. Optionbutton = 1 of a group of choices; Checkbox = Select all that apply. FWIW.
