VBA Code for Button to Disperse Data to Respective Sheet

I am working on an excel file for my wife and her job and I am entering data in a master sheet and need data to be entered and then a button be clicked and it puts the data into the respective worksheets for the facilities. Or if there is a formula for this as well would be helpful. I have attached a generic redaction of the file as she is in healthcare and I cannot share the information. Any help is appreciated. This is giving me a migraine lol. Thank you.

That works perfect! So my next question is How do I adapt this when I change the facility names to their respective Facilites? I have 5 different facility names obviously.

So if I go to enter data and hit the button to Submit Data, it transfers it correctly. If i go enter another line of data and hit Submit Data it duplicates all previous values entered as well.
