Macro for transforming columns with data

I have the file Input.xlsx. I want to create a macro that changes the name of the columns, set the data in the right way as shown in Output.xlsx. Also the columns in red are added with the defined cell data.

How can one combine this in a macro? In this example it is with a small data set, in general I have a more complicated data set but with the same columns name in both examples.
