Return Multiple Matches in Single Cell Based on Multiple Criteria.

I have attached example sheet in which i am able to get the single result. by viewing formula of L12 and N12 you will be understand the which ranges needs to lookup.

I expected multiple results in same cell with comma separated manner. and if nothing found return blank. You are requested to provide the code so i can flexibly used in different workbooks and different references. Also make sure that if so in bigger database and more number of results are returned in same cell if case may be.
