Anyway, there is not sufficient information here to provide an answer except to look at UserForm examples, eg the databaseform here

sorry I should have been more specific ... all i wanted is for a user-form to appear where users can put their name, country and the feedback they want to leave and then press "OK" and the feedback will be send to my email address..
