Count Pivot table (more to it)

I built a pivot table from a set of data, I used two grouping and with a total. Let's say the grouping is like...

ID_Number, Med_name and the count vaule is the total of whichever. Aside from that I have a vlookup function that compares the med_name to another list of medication (that's outside the pivot table). The result is either, Don't Count, 1st gen anti-psychotic, 2nd gen anti-psychotic and no match. How do I create a function that picks up the ID_Num and check how many of 4 things they have. So let's say I have Luvox as don't count. Zyprexa and Zyprexa 15mg as 2nd psychotic (but I want to count them as one not two). sorry its pretty difficult to explain

OK attached a file that look somewhat similar. Two tabs, med and anti-psychotic. I want to count how many 2nd gen anti psychotic I have, but instead of counting two for Zyprexa/Zyprexa 15mg, I want it to count as one because they are the same med.
