Code for a Clear Form Button

Hi, I am working with excel and VB 2003 and trying to code a clear button for my userform. I just can't seem to get it to work....
I am also looking for help making a code to print the filled out user form before i submit the data to my spreadsheet. I have attached my work for review..
Any help that can be given on either matter is greatly appreciated!
