VB challenge

I don't know if this is just plain cheeky, and if it is please tell me, but I would like someone to make a form for me with VB please.

I know nothing about VB, except that it can be used to make a form. I want one made from the attached xls file in the form of the attached doc file.

Mmm - I'm editing this message because I forgot to attach the files and now I find I can't attach ANY file. The icons don't work for me. Anyone tell me what I'm doing wrong please? Can't see a Browse button anywhere.......
