UserForm exiting prior to completion

The userform I created exits when you press the "ok" button even all the fields are not complete. I know that this is because of where I placed the "unload userform1" command, but I cannot think of how to fix it. I want the first 5 text boxes I have to be mandatory and required to be filled in while the last 2 do not need to be filled in. I will attach the sheet.

I would like to get rid of the "enable macros" prompt without deleting the modules. How can I do this?
