Data Input Form using Command Button

I need to create a form that allows Data entry based on selected Criteria.

The form requires the user to choose the STATE, DATE, and DEPARTMENT from a drop down menu.

Once those are chosen, the user goes in and enters 4 additional variable.

Upon hitting enter, the entered variables will be transferred to the correct worksheet based on the state chosen, and within that worksheet it will go into the correct date and department.

I have the template set up, and I entered in IF THEN statements. However I need to program the ENTER button to transmit that info to the correct cell.
