user form to add records to rows

I have created a sheet where I have some categories running vertically. I am trying to creat a user form where someone can launch the form, select the item they want to add a record to, and upon hitting enter it will submit that value after the previous record in the corresponding row.

I am attaching an example.

Thank you for your help. I have been tied up so havent had a chance to check in, but going to go through your help tonight and see what works and mark the threads accordingly.

I am not sure that entirely helps me. I need to be able to search a column verticaly and if it matches that field I need it to add that record in that row after the previous entry. I am not sure if the example you presented does that or not.
