Create User form to pull from equipment list and fill in data

Can anyone show me an example of a Userform that would fill in data based on equipment list.
Choose a manufacturer - have a product list that changes depending on the manufacturer - Add the model to a list and create another list you can choose multiple accesories from. Fill in the info and then add another.
I am attaching an example 2 manufacturers w/ models to choose from.
    Thanks

three lists 1. o1:o4 lists the manufacturers 2. secondary list on SheetMFG1, sheetMFG2, etc... 3. Accesory Lists: sheetModel1...
That is where I struggle - how do you point the combo boxes to different sheets (if MFG1 is chosen then list from sheetMFG1 etc...)

Your workbook is not very well designed to make you application easy to code. I can't really follow what you need in the comboboxes, and your lists on the model sheets will be difficult to use because they contain empty rows.
