    Formula in Form

I would like to know how to create a formula with a User Form. Attached is an information form. I would like to have a couple of formulas to calculate a date, date issued field + 5 years in Expiration Date Issued field. Also in the Years Valid field I would like to have the number of years left based on Expiration date field. A certificate is only good for 5 years after the issue date.

It would be easier to work if yo named your TextBoxes, this is a good practice to start.

    See if this helps

    Re: Formula in Form

The Dates work great. How do I make it a short date and it must end Expiration date should be 6/30/???? 5 years later. I'm sorry for not explaining it correctly the first time. An example 1/7/2009 issue date, 6/30/2014 expiration date. in the years valid date a decrease in years like in the example 2010 would be 4 , 2011 would be 3.
