    Question Macro to Autocreate New Sheet

    Is there a way to auto create a new sheet from a template using a macro? I have a button that indicates whether or not the user had a problem with the application, and when they click it, I want a premade sheet to pop up and ask for information on the problem. I know I'd probably need to create a template for the sheet, which I'm not sure how to do either, nor how to make a call for that sheet to auto create.
    Hopefully I've made myself clear enough for someone to answer my question. If not, let me know.

    You create a form in the worksheet for the user to answer. When finished I would suggest that you hide it
