Adding row automatically

Hi,

Now I'm making a form that will trigger questions from other sheet.

now the form is a static form i have to convert it into dynamic one.

the question is how the cells for questions will be add automatically.

hurm there are 5 departments.

every department will be asked by different question.

eg : department A will be asked by 5 question, department B will be asked by 7 different questions. these question are in worksheet B.

how i can call the question to be appear in worksheet A according to the department ?

How does the form know what questions to ask? Or is there a different form for each sheet?

If there is only 1 form then:
How is the form called? Is there a button on each sheet that calls it? Is there a menu selection that calls it? Or how?
