getting data from another sheet in a workbook

Hi guys, I'm working on the sheet entitled "cover page". But the data i need to fill-in some fields in the cover page comes from the sheet entitled "database". When i call the userform from the cover page, it produces an error but when i call it from database, it works fine. How to solve this problem?

this is the code i use to initialize the values in the listbox wherein the values come from "database".
