Moving data to create a header

I have an EXCEL file that is converted from a PDF. I need to use this EXCEL file to import into a database. Unfortunately the two key data, StudentID and Name is not a header when converted to EXCEL. I have to import 2400 records but I need the EXCEL set up right first. I have the file attached. Sheet1 shows how it comes to me formatted. Sheet2 is how I need to have the final product. Any help would be appreciated. I have been working on formulas to try and move the data to new columns but no success. Thank you.
