Array Formula if 2 fields = display 3

I have 2 sheets 1 & 2. If the date and RM from sheet2 matches date & RM in sheet1, I want to display the type, first and last name.

The formula always finds the first column on sheet 2 and displays it correctly. The other columns are never found even though the formula auditing evaluation looks as though it will insert the text - 2 values evaluated to 1 (true). Spreadsheet attached.

the vlookup does an excelent job matching the RM but it leaves out the date match and doesn't pick up the first column of sheet2. I don't understand the repetition of the formula with the "" between the repetition.
