Command buttonto populate form

Looking for code that can be used to fill data from spreadsheet to another form in a spreadsheet. Please see attached.

I want to be able to click a command button on side of the line of information required and that line will populate the main form.

assign the button to this macro
Select B4 on Sheet61, then click the button
