I have a workbook and i want to rearrange data (a huge list) from sheet1 into sheet2, i tried but i can not do it. could anyone to help me, please?

I assume the data in real file differs from the one in example, am I right? What do you mean by "adapt the code"?
The code works fine on example file.

yes, it does a little bit! is my mistake i did not copy very well the data into test file.

thank you very much for your help, it works fine!
I have a small issue, the data is sent to a user form, but if there are 2 or 3 people with the same last name, in the user form
is displayed only one.
is there a way to distinguish between them by midle name or first name and to have the possibility to choose (from combobox) if the first one is not what i'm looking for?
i attached a file.
any help is really appreciated!

I reduced the unnecessary complexity of the userform.

There's one more thing:
to arrange data fom sheet 1 to sheet 2:
first delete column E in sheet1
