Adding additional columns and values based on the given data

Hello everybody,

This forum helped me a lot in converting huge amount of real data. I thank everyone who helped me. This is the last data that I need to convert.

Try this. Before you run it, add a sheet called Target with headings on row 1 (A1 = "No.", B1 = "Days", C1 = "Weight")
