Code to Copy & Paste Transpose Selected Information

I would really like the code for the following attached example as I run this report daily.

The Test tab is the data layout I currently have and the outcome tab is the data layout I would like to have.
Basically, I always need the data in columns A-E and then I need the date associated with each visit number to be in one column each - one for visit number and one for visit date - listed vertically, instead of across multiple columns.

NB: that the in each of the visit columns (F-H) in the Test tab, I only need the data upto the last date logged for that visit. So, for e.g. visit 1 dataset = columnA:E& F, Rows 6 to 21, Visit 2 dataset = column A:E&G, row 6 to 20 and Visit 3 data = column A:E&H, row 6 to 19.
Also, the visit numbers increase everytime I run the report and can go up to visit 25.
