Format Change in Data

I pulled some grade data by term, instructor, and course with SQL script from BANNER ERP system. The data format is as in the raw data sheet in the attached excel file . But I need the data to be in a summary format like in the format needed sheet in the attached excel file. I cannot get the report with SQL in this format. Can some one help me with suggestions on how to switch the report format in excel? Thanks.
