match or index multiple criteria

In my sample;
in cell AM6 in need a formula that will do the following.
If J6 is blank then AM6 is blank, if not, match F6 to (pricing) M74:M305, match V6 to (pricing) N71:AA71, match W6 to (pricing) N72:AA72, and match AM5 to correct product in (pricing) grid.
I know index should be in there somewhere but havent had any luck.
I left my incorrect formula in AM6.

that works. I had to modify it a bit to work for additional cells and fix a few errors in my grid but it works great.

I need some help with another formula with this same sample. I think it will be close to the same as the one given to me by NBVC but with different results.
