How to Create User form which appears upon opening workbook?

I need some help creating a User form which initiates upon opening an excel workbook. There are cells fields that I want to populate with the information from this user form. I have attached what I would like the user form to look like.
Can anyone please help me code this form to work properly. I can provide more details if needed. Thank you!

Thank you tiger. I have a few more questions that would help me perfect my form.

I have attached my file with current coding.

Basically, I want the initial form to be filled out, "ok" clicked, and designated cells be auto filled with the information inputted on the form. Also, I want the initial user form only to pop up the first time the file is opened.

If you can please help further, it would be much appreciated.
