Create new sheets based on list and transfer data to correct cells and can be updated

Here's what I'm looking for. I track the giving at our church and want to simplify my job, duh.
My 1st sheet (named "Givers")will have data manually input every time there is a new giver. Row 1 will be the headers so data will start in row 2. Column A will be the name, B the street address, C city state zip, D phone number.
My 2nd sheet (named "Blank" is a blank copy of our giving record page that has a block for every weeks contributions for that giver (this sheet is updated with formulas I've already made and use).
Every time I add a new giver I want to run a macro that will ignore the givers its already made sheets for, copy sheet 2 "Blank", change the sheet name to the name from column A of the new row, and then copy column A of that row to the new sheet cell B10, column B to B11, column C to B12, and column C to B13.

Thanks in advance for the help. I'm pretty good with formulas but not so much with macros.
