Repeating Vertical Data consolidated into Horizontal data?

Can anyone suggest a formula that would take the data presented below into a consolidate single line item with the data presenting in a horizontal time ordered way? I have tried using MATCH & INDEX & VLOOKUP but I am not an Excel guru and am not sure if its even possible.
