Excel Macro/VBA to Automate Outlook Calendar Appointments

I realize that this question may have already been solved, but I am truly having difficulting with the VBA Code.

Backstory: We have files that require 90 day and 120 day reports from the date the file was opened. Currently we are manually entering the data in multiple locations, e.g. wasting man hours.

Desired Outcome: I would like to be able to imput the data in an excel spreadsheet (we do not have Access) and run a Macro that will automatically create the "meeting/appointment" for the Report Due Dates.

I have attached a sample workbook, but in case I did it wrong the jist is as follows:

    A B C D E F G H
1 Client Name File Opened 90 Day 120 Day Start Time Duration Paralegal Attorney
    2 Smith, Tom 4/1/11 6/30/11 7/30/11 8:30am 15 min Paralegal1 Attorney1
    3 Rich, Ivana 8/1/11 10/30/11 11/29/11 8:30am 15 min Paralegal 2 Attorney2

    TIA for your help!!
