text box for all listbox values

Hi,
I need to create a textbox for each of the values in the listbox. Example... select Abrasive in combo1 followed by belt in combo2. I need to allow the user to enter a value for each of the characteristics in the listbox. I then need to concatenate the values in the textboxes in a specific format.

Any help is greatly appreciated.
