Data Entry box upon opening

I would like to have a combo box open on start of my workbook which takes the information for a new flight and enters it into the next available row of my existing pilot's logbook. I need a field for date, type and registration of aircraft, crew and flight time, plus a drop box to organize the time into the right category. Any suggestions would be appreciated. I have attached the workbook for review if it is helpful.
