Copy Data loaded via Userform to a worksheet named in 1 field of the Userform

Hi every expert, need some help.
I have a WBook with 10 Wsheet named 1, 2, 3...10 and a UserForm with 5 different fields....Field 1=WSheet name...Field 2=Date...Field 3=Name...Field 4=Address and Field 5=Telephone...I want to copy the data (Field 2 thru Field 5) in the first empty row in Wsheet entered in Field 1.

How about an example file. Your description is a bit broad as you don't advise if your form has textboxes, listboxes, comboboxes etc. Any reply would have to be extremely broadbrush and may not give you enough to modify to suit your specific situation.

WSheet 3.....A1=45....B1=56....C1=76...D1=88...then
in Wsheet 3 (Because Field 1=3), will have in row 2....
A2=2/2/2012.....B2=Rylo......C2=34 Gate Rd and in D2=7656545
Sorry, but I haven't created a workbook yet...

Thank Rylo for following my inquire.....it will be 3 control buttons....Execute...Clear...and Cancel...My data will be only text boxes

OK, so something generic, assuming that the textboxes are named textbox1, textbox2... textbox5
